End-of-Life Planning Handbook for Carers

End-of-life planning for carers: Passing the Torch of Care: A Legacy of Love for the Next Caregiver

Caring for a loved one with a disability or special needs at home? This kit is the perfect way to document all of the important information needed to ensure their continued high quality care, upon your passing.

What would happen to the care of a loved one who relies on you if you weren’t around?

“I know when I was caring for my mum with dementia it was a huge undertaking – appointments, medications, keeping to familiar routines, foods she liked, keeping her calm with her favourite stories and music. All those years ago, I never gave it a second thought that I wouldn’t be around to care for her. Whilst she is no longer with us, I am grateful that I could be her safe place, someone she knew she could rely upon, who loved and cared for her in a way that she needed.” – Nat, Founder, Handbook of Your Life

The Carers Kit: Dual Purpose

The Carers Kit was designed for end-of-life planning, but it really has a dual purpose. How?

The Carers Kit is broken up into 5 distinct parts. One of these parts is about your caregiving duties (Part 3). This section can be used by your loved ones after you pass. However, it can also be used while you are still here as a carer, in the following ways:

For Support Workers and Paid Carers

You can utilise Part 3 of the Handbook, the Caregiving Duties section, to comprehensively record your loved ones care, ready for use by a support worker or paid carer. Additional information pages are available to ensure continuous documentation of care providing, and these new notes can be seamlessly added into the relevant sections of your Handbook.

Utilising your Handbook for use with an external carer, means that you know that they are fully aware of all of the intimate details of your loved one with regards to the continuity of their high quality care.

Respite Care

Part 3, the Caregiving Duties section of your Handbook, can be sectioned off, printed out and provided to a respite centre or family member taking over care for your loved one for a short time, should you need to take a break. It can provide a means for someone to easily come in and pick up where you, as the carer, left off. This provides you peace of mind that the individual needs of your loved one have been concisely documented so as to give the respite centre or family member all the information they need to continue the highest level of care to your loved one – just like you do. This takes the weight off your shoulders in that you have left no stoned unturned in documenting exactly what other carers will need, in your absence.

The life of a carer is such an incredible commitment that you would want the high standard of care that you provide to your loved one maintained, in the event of your passing or absence. Routines are so essential, as can be special diets, exercise regimes and keeping familiar people around so that your loved one isn’t scared or worried about what is going on. Continue to be their voice, even in your absence.

This Handbook allows one individual to document their life, in conjunction with documenting the life of the loved one that they care for.

The Handbook

Are you caring for a family member with special needs or a disability?
